2) Medical Care & Eligibility
- All treatments are provided only after appropriate evaluation by a licensed provider. We may decline or modify treatment for safety or clinical reasons.
- Results vary; no outcome is guaranteed. Information on our website is educational and not medical advice.
- Patients under 18 require guardian consent and may be ineligible for certain procedures.
3) Appointments, Cancellations & No-Shows
- Please provide 48 hours notice to reschedule/cancel to avoid fees.
- Late arrivals may be shortened or rescheduled; no-shows may incur a fee or forfeit deposits/credits per policy.
4) Pricing, Payments & Promotions
- Prices may change without notice. Quotes may exclude taxes, pharmacy, and lab fees unless stated.
- We accept major cards and other methods shown at checkout. Third-party processors handle payment data.
- Promotions/coupons are limited-time, non-transferable, and not combinable unless noted.
5) Memberships & Credits
- Memberships are month-to-month; please provide 30 days notice to pause/cancel.
- Wrinkle Relaxer members receive $12/unit pricing (save $1/unit), 10% off other services, and a $40 Jeuveau® rebate added to their account every 3 months.
- Credits accrue monthly, apply at service, and roll over while membership is active. Credits and benefits are non-transferable and may not combine with some offers.
6) BHRT & Weight-Management Programs
- BHRT Membership: $149/month. Labs and prescriptions are paid out-of-pocket. Initial consult $199 (credit may apply during limited-time promos).
- Weight Loss Program: $297/month includes lowest-dose medication (~$90 value); dose/pharmacy pricing may change medication costs.
- Combination Membership: $199/month includes BHRT plus GLP-1 coaching; medications billed at pharmacy cost.
7) Refunds & Returns
- No refunds on services rendered. Product returns follow manufacturer policy and condition requirements.
- Prepaid packages/memberships are non-transferable and subject to expiration terms where applicable.
8) Photos, Testimonials & Educational Content
- With written consent, we may use de-identified photos/testimonials for education/marketing. You may revoke future use via written request to [email protected].
9) Third-Party Pharmacies & Labs
- Meds are dispensed by independent pharmacies; we do not control availability or pricing.
- Lab services may be billed directly by the lab; you are responsible for those fees unless stated otherwise.
10) Limitation of Liability
To the fullest extent permitted by law, we are not liable for indirect, incidental, or consequential damages. Total liability shall not exceed the amount you paid for the service at issue.
11) Indemnification
You agree to indemnify and hold us harmless from claims arising out of your breach of these Terms or misuse of services.
12) Governing Law
These Terms are governed by the laws of the State of Oregon.
13) Changes
We may update these Terms at any time. The “Effective date” above reflects the current version.
